Charleston, WV – Are you passionate about making a real difference in the lives of others? Jobs & Hope WV is seeking a Transition Agent in Kanawha County/Charleston, WV to help individuals transition through recovery and reentry, preparing them for successful educational and career opportunities.
In this impactful role, you’ll assess participants’ needs, develop personalized plans, and connect them with resources to overcome employment barriers. By partnering with businesses, community organizations, and support services, you’ll be instrumental in helping participants achieve self-sufficiency and career success.
The ideal candidate will have a minimum of an associate’s degree in education, social work, or human services, or equivalent work experience. Strong interpersonal skills and the ability to work within a multi-disciplinary framework are essential.
